Service Description: A registered charity that offers residential placement for youth 15-18 years not able to live with family but not ready for independent living

MAJOR SERVICES: 

- Residential
  • 8 bed, co-ed group home for adolescents
  • if family re-integration is not possible, the adolescent is prepared for independent living or another alternative

- Non-Residential
  • community consultation service for adolescents and their families who are experiencing difficulties
  • provide referrals * case-management * community liaison * on-going support to former residents

- Transitional Support
  • provides support and case management to adolescents and their families if involved, who are making the transition from children's services to adult services or are leaving residential programs to live in the community
  • support includes and is not limited to finding employment, housing, planning for post-secondary education and any other support as needed
  • also provides outreach to referral clients who are not suitable for the residential program

- Life Skills
  • all residents are required to take part in bi-weekly informal workshops
  • the objective is to build life skills so that residents are able to successfully transition into independent living
